TRIBAL ASSISTANCE IN HEALTH SERVICES
The Caddo Tribal Council has appropriated funds from the motor fuel monies to assist tribal members in health services for the fiscal year beginning each October 1, and ending September 30. Assistance is a one-time-only, per Fiscal year for eligible enrolled members, the limit will be $100. Dental, prescription, and medical bills must be current, for a period of time, after October 1.
The categories which are approved for assistance are:
Eyeglasses may be obtained at any optometrist and any amount over $100.
Applications for assistance may be obtained from the Caddo Tribal Office and Health Department or you may download an application below.
Completed applications, along with the original bill and a copy of your CDIB or enrollment verification must be mailed to the:
