The CDIB card is a federal government card certifying an individual's blood degree and tribe. The CDIB is approved and signed by a Bureau of Indian Affairs representative once they establish the individual is a descendant, by the blood of a Dawes Enrollee. The Tribal Membership Card establishes an individual as a member of the Choctaw Nation of Oklahoma and can only be issued once a CDIB has been approved.
Relinquishment is a notarized statement used to relinquish your membership/citizenship rights with one tribe in order to become a member/citizen of another tribe when you possess a blood quantum from more than one tribe. The Choctaw Nation of Oklahoma does not allow dual-enrollment. Listed below are the relinquishment used by the 5 Civilized Tribes. If you are a member of another tribe not listed, please contact the Membership/Citizenship department of that tribe.
